CBD Oil and Immunotherapy

CBD Oil and Immunotherapy: Safe Together in Cancer Treatment?
In recent years, two very different yet widely relevant topics have come up more frequently in oncology: immunotherapy, one of the most promising cancer treatments, and CBD Oil, which an increasing number of patients use as a complementary approach to alleviate symptoms.
But what happens if both are used simultaneously? Could CBD weaken immunotherapy, or on the contrary, help the body heal?
This question was examined in a recent study that sheds new light on the connection between cannabis and the immune system.
What is Immunotherapy?
Immunotherapy (also called checkpoint inhibitor treatment) works by activating the body’s own immune system against tumor cells.
Drugs such as pembrolizumab, nivolumab, or ipilimumab release the “brakes” on T-cells — enabling them to recognize and destroy cancer cells.
This has been a major breakthrough for many types of cancer (e.g., lung, melanoma, kidney), but because it “overdrives” the immune system, it often causes immune-related side effects.
Why Has There Been Caution Around CBD and Immunotherapy?
The goal of immunotherapy is to “awaken” the body’s natural defenses and prompt the immune system to identify and destroy cancer cells.
By contrast, cannabis—and thus CBD Oil—affects immune cell function, particularly through the CB2 receptors. This connection has been known for some time, and certain laboratory studies suggest cannabinoids can “calm down” the immune system in some cases.
It’s understandable why many long assumed cannabis or CBD Oil might reduce the effectiveness of immunotherapy treatments.
Some earlier Israeli studies even suggested that patients using cannabis had worse outcomes when on immunotherapy.
However, these studies were very limited: few patients, different treatment backgrounds, various cancer types — so the results were not reliably generalizable.
New Research, New Results
A recent large-scale study now sheds fresh light on the question.
Researchers examined 201 patients with non-small cell lung cancer undergoing immunotherapy (checkpoint inhibitor treatment). Half of the group also received cannabis in the first month, the other half did not. Comparing the two groups revealed: no difference in survival rates or disease progression.
The study was complemented by animal experiments. Various treatment combinations were tested in mice: only immunotherapy, only cannabis, both together, or neither. The result?
Cannabis did not reduce immunotherapy effectiveness, and mice receiving only cannabis even showed slightly slower tumor growth compared to controls.
The researchers’ conclusion is clear: cannabis — and thus likely CBD Oil as well — does not diminish the effect of immunotherapy in cases of non-small cell lung cancer.
This does not mean the same applies to every type of cancer or all therapeutic combinations, but it is certainly reassuring news for those worried that CBD Oil might “harm” their treatment.
What Does This Mean in Practice?
Many patients use CBD to ease the unpleasant side effects of their treatments.
Most Common Side Effects of Immunotherapy
Fatigue and Exhaustion
Occurs in up to 60–80% of patients. This is not simple “tiredness,” but persistent, deep energy loss that sleep or rest does not easily relieve.
Skin Issues (rashes, itching, dryness)
The immune system’s overactivity often attacks skin cells as well.
Gastrointestinal Problems (diarrhea, nausea, loss of appetite)
Immune reactions in the gut can cause inflammation (so-called immune-mediated colitis).
Joint and Muscle Pain
Autoimmune inflammation can affect joints and muscles, causing symptoms similar to arthritis pain.
Sleep Disorders and Anxiety
Along with physical strain, psychological stress also increases, making it hard for many patients to relax, fall asleep, or stay calm during treatment.
Liver, Lung, and Thyroid Inflammation
Less common but serious side effects when the immune system attacks internal organs.
How Can CBD Oil Help Alleviate Side Effects of Immunotherapy?
CBD Oil (cannabidiol-containing extract) is not a medication, but according to several studies, it can effectively support quality-of-life improvements alongside immunotherapy.
Let’s take a closer look at how it may help with specific symptoms:
Fatigue and Sleep Disorders
CBD Oil can balance the sleep cycle, especially when sleep disorders accompany fatigue.
Studies suggest CBD reduces nighttime anxiety and promotes deeper sleep, helping patients wake up feeling more refreshed.
Important: CBD is not a sleeping pill but supports the body’s natural rhythms.

Anxiety and Stress
Immunotherapy is a long and mentally taxing process.
CBD Oil acts on the brain’s serotonin receptors (5-HT1A), which reduces anxiety symptoms and supports emotional stability.
Clinical trials have shown CBD to be beneficial in social anxiety and post-traumatic stress disorder — effects relevant to cancer patients as well.

Pain and Inflammation
One of CBD’s best-known effects is its pain relief properties.
Through its anti-inflammatory effects, it may reduce joint or muscle pain caused by immunotherapy.
Activation of the endocannabinoid system can help regulate pain perception.

Skin Problems
Thanks to its antioxidant and anti-inflammatory properties, CBD can soothe the skin, reducing itching, redness, and dryness.
Topical CBD creams can also complement medical treatment for mild skin issues.

Digestive Issues, Nausea
Like chemotherapy, immunotherapy can cause loss of appetite or nausea.
Clinical studies indicate that the combination of CBD and THC (for example, in the form of full-spectrum CBD Oil) can alleviate nausea and improve appetite.
CBD’s effects on the brain’s vomiting center and serotonin receptors can help reduce queasiness.

Supporting Immune System Balance
CBD has immunomodulatory effects — meaning it may help restore balance if the immune response is excessive.
This can be especially important for immunotherapy-related inflammation (such as colitis or skin inflammation).
Practical Advice If You’re Receiving Immunotherapy and Using CBD Oil
-
Talk to Your Oncologist!
Don’t hide if you’re using or planning to use CBD Oil. Your doctor can assess potential interactions. -
Choose Products from Reliable Sources.
Only purchase oils with laboratory-verified, clearly stated CBD content. Uncertain-origin products can be not only ineffective but also risky. -
Start with a Low Dose.
CBD Oil’s effects vary individually and depend on the state of your immune system. It’s always best to begin with a small dose and observe your response. A comprehensive dosage guide is available here → https://www.cannadolcbd.eu/hu/cbd-adagolasi-utmutato -
Don’t Consider CBD a Substitute for Medication!
Use it only as a complementary option, under medical supervision! -
Maintain Regular Monitoring.
If you’re receiving immunotherapy, track treatment effectiveness and any side effects jointly with your healthcare provider.
Summary
The relationship between cannabis and immunotherapy was uncertain for a long time, but recent research shows that the two together are not necessarily a dangerous combination.
While further studies are needed, current evidence suggests that CBD Oil can be safely used alongside immunotherapy if applied cautiously under medical supervision.
CBD Oil is not a miracle cure and does not replace oncology treatments, but it can be a valuable complementary option for relieving treatment side effects and improving daily well-being.
