One of the most significant was a Phase III clinical trial published in Nature Medicine in September 2025, testing a full-spectrum, high-CBD hemp extract (referred to as VER-01 in the trial) on patients suffering from chronic lower back pain.
This large study included 820 patients (394 received the active substance, 426 received placebo) and treated them for 12 weeks. The results showed that taking VER-01 reduced pain intensity by an average of 1.9 points (on a 0–10 scale), whereas the placebo group experienced a 1.4-point reduction — the difference between groups was about 0.5 points, which was statistically significant (p < 0.001).
At first glance, this may seem like a small difference, but the study also reported that a higher proportion of patients receiving the CBD extract achieved clinically meaningful pain reduction (e.g., at least 30% or 50% reduction) compared to placebo. The preparation was particularly effective for pain with a neuropathic component: patients with nerve-related pain showed much better symptom improvement from the CBD extract (an average decrease roughly 7.3 points greater than placebo, p = 0.017).
The treatment also proved effective in the longer term: during an open extension phase, patients taking it for 6 months saw an average pain reduction deepen to 2.9 points, with the effect sustained. The side effects were somewhat more frequent with the full-spectrum cannabis product compared to placebo, but were mostly mild and temporary (e.g., drowsiness, dizziness); no dependence or withdrawal symptoms were observed.
The study concluded that the VER-01 full-spectrum extract is a safe and moderately effective new treatment option for chronic lower back pain. Since this is a whole-plant extract (containing traces of THC and other cannabinoids and terpenes), the results suggest that the potential pain-relieving effects of CBD may be enhanced when combined with other components (entourage effect). However, the authors emphasize that much depends on the precise composition and dosing of cannabis-based products.
